Deliver a check for $275 made out to “BSA Troop 28” to 2104 Davis Drive, Burlingame, CA 94010 or bring to the next troop meeting.
Form B - two pages need to be filled out. This form asks for a list of current immunizations and the front and back of the current year's insurance card.
Form C - Doctor's signature and 2019 date.
Mail or deliver TWO (2) copies of each form, immunizations, and front/back of insurance card
4 copies of shooting form if you want your son to be able to shoot at camp
